Behavioral Health Professional Supervisor/ACT Team Leader

 Salary $67,080 to $81,536 FLSA Exempt

I.                QUALIFICATIONS:

 Education/Experience

1.               Licensed Practitioner of Healing Arts including: LCSW, LMSW, Psychologist, Physician Assistant, Nurse Practitioner, Registered Nurse, Mental Health Counselor, Marriage and Family Therapist, Psychoanalyst, Creative Arts Therapist required                                                        

2.               Minimum of three years’ experience working in Behavioral Health

3.               Supervisory experience required.

4.               Preference given to individuals with experience on multi-disciplinary Team.

Skills

1.               Knowledge of psychodynamics and psychopathology of social factors affecting disease and disability

2.               Good Knowledge of principles and techniques of psychotherapy

3.               Excellent organizational and time management skills

4.               Ability to work within a multi-disciplinary team approach to outreach clinical/case management model

5.               Knowledge of Psychopharmacology

6.               Diagnostic and assessment skills

7.               Excellent Leadership abilities with strong supervisory skills

8.               Excellent oral/written communication skills

9.               Knowledge of community agencies and resources

10.            Familiar with computer programs and electronic health Records.

11.            The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.

12.            Ability to legally operate a vehicle in NY State

II.               RESPONSIBLE TO:    CHS ACT/Care Management Director

III.               MAJOR FUNCTIONS:

A.        Clinical/Program

1. Provides leadership and participates as part of the clinical team in pre-admission, admission, discharge, assessment and planning processes and completes initial service assessments.

2. Responsible for case record maintenance and other data in accordance with program policy and requirements.

3. Provide crisis intervention services as necessary.

4. Serves as service coordinator for 4-6 clients, completes comprehensive assessment, engages clients in goal identification/service planning, oversees case management and documents in a client chart.

5. Provide individual, group, and family therapy as needed

6. Work with ACT Team psychiatrist/PNP and coordinates treatment, as needed.

7. Maintain required documentation and deadlines

8. Participate in and successfully complete all required staff training and development.

B.     Administrative/Supervisory

1. Provide direct (individual and group) supervision of each specialty role based

on evidence-based practice, trauma informed care, cultural humility and person-centered practices.

2.Provides administrative oversight to all program requirements based on Flexible ACT model and OMH requirements.

3.Oversee QI activities, reviews and signs off on treatment plans and monitors records to ensure incorporation of client-centered practices.

4.Oversee assignment of weekly and weekend after hour coverage

C.     Catholic Charities Agency & Community Related Activities

1. Participates in County Single Point of Access (SPOA)meetings.

2. Coordinate with local service providers to ensure smooth transitions of services.

3. Attend relevant educational seminars/trainings.

4. Attend Flex ACT Team Leader meetings, Assisted Outpatient Treatment (AOT) and other relevant professional meetings.
