Teen Transitional Living Program (TTLP)
Care Manager Level 1
Grade 8 (Non-exempt) FSLA Salary $19.95-$22.45 per hr.
I. QUALIFICATIONS
A. Education/Experience:
1. Bachelor’s degree in Human Services or related field preferred
Two years experience in adolescent/family services or equivalent
OR
Equivalent combination of education and experience
B. Requirements:
1. Good oral and written communication skills
2. Demonstrated leadership ability
3. Good knowledge of community resources for adolescents
4. Demonstrated ability to establish rapport and relationships with clients.
5. Good interpersonal skills
6. Valid NYS or PA driver’s license and ability to drive agency vehicle
7. Must be able to lift over 40 lbs.
8. Computer skills adequate to job duties
RESPONSIBLE TO: TTLP Supervisor
II. MAJOR FUNCTIONS
A. Administrative:
1. Participate in staff meetings and agency trainings as required.
2. Complete 40 hours of trainings as required by OCFS.
3. Participate in on-call rotation.
4. Participate in monthly supervision with program supervisor.
5. Responsible for case record documentation and daily and monthly record keeping.
6. Represent the Agency and Program in a positive manner.
B. Client/Case Management Functions:
1. Maintain daily face to face contact with clients.
2. Complete daily apartment safety checks.
3. Conduct monthly fire drills with clients.
4. Complete spill kit and first aid kit checks monthly.
5. Participate in admission and discharge planning.
6. Link clients to necessary services and act as an advocate.
7. Complete individual needs assessments including an independent living assessment.
8. Develop a person-centered service plan with the client.
9. Provide skills training in shopping, money management, menu planning, transportation, leisure skills, personal hygiene, housekeeping, use of community services and any other IL skill training need.
10. Provide education and information to pregnant and parenting youth
11. Ensure that residents are safe and following program rules and expectations.
12. Ensure environmental safety in TTLP residences.
13. Meet with clients to update and review service plan progress.
14. Intervene to resolve crisis situations.
15. Provide transportation as needed.
16. Assist clients in setting daily/weekly activities
17. Help clients move into and out of the program.
18. Coordinate with other service providers and attend meetings as necessary.
19. Co-facilitate weekly ILS Group, community service projects and program activities.
20. Provide Transition Planning and After Care services.
