FLSA Exempt Annual Salary $62,608-$76,107
I. QUALIFICATIONS
A. Education/Experience
1. NYS Registered Nurse License, BSN preferred
2. Two years professional nursing experience
3. One year experience with DD/MH population preferred
B. Skills
1. Interpersonal, organizational and communication skills
2. Ability to instruct staff and individuals in basic health care
3. Ability to work with the Interdisciplinary Treatment Team
4. Familiar with community agencies and resources
5. Familiar with computer programs (e.g. Word, Excel, EHR)
6. Ability to legally operate a vehicle in NYS
7. The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighting up to 40 lbs.
II. RESPONSIBLE TO: Supervising Nurse/Care Manager
III. MAJOR FUNCTIONS
A. Primary Duties
1. Work as part of a Care Coordination team
2. Assist/cover for other nursing staffs absences
3. Provide medication education to staff and individuals
4. Participate in multidisciplinary team meetings and case reviews
5. Identify and advocate for individual rights
6. Teach/assist in developing/maintaining ADL skills
7. Teach medication administration/supervision to staff
8. Certify DD staff to pass medications
9. Conduct initial and ongoing assessments
10. Monitor routine health care of all individuals
11. Provide/arrange for routine and emergency healthcare
12. Review all health related incidents
13. Oversee/order all medications
14. Maintain health records according to standards and regulations
15. Provide in-service training regarding health issues
16. Maintain/coordinate effective and on-going communication with other service providers
17. Participate in the development/documentation/review and update of care plan
18. Act as linkage to community services
19. Coordinate and conduct care planning activities for a designated case load of recipients
20. May utilize health information technology to coordinate care and communicate with providers
21. Act as medical resource consultant for team members
22. Promote health and wellness activities for recipients via individual or group sessions
23. Provide crisis intervention services as necessary
24. Provide transportation as needed to necessary services
25. Promote self-advocacy and personal empowerment
26. May be assigned to participate in rotating afterhours/weekend on call coverage
27. Establish relationship with all individuals, family members, and advocates
28. Provide blood borne pathogens and other training to program and agency staff as needed
29. Understand and follow all program policies and procedures
30. Represent the agency and program in the community and with other service providers
31. Participate in and successfully complete all required staff training and development activities
32. Regular and substantial contact with individuals receiving services
33. Perform other related tasks as directed by supervisor
